SALARY RANGE : $33,241 - $41,552 - $49,862 GRADE: 68

 

JOB DESCRIPTION:

The Community Resource Court Program is hiring immediately for a full time Care Coordinator position. The Care Coordinator will provide support to the Orange, Person and Chatham County Area Program and the criminal justice system in a variety of functions. The tasks performed by the Care Coordinator include assessment, accessing care, liaison services and case management. The Community Resource Court Care Coordinator will conduct assessments with consumers in the court, jail or community focusing on emotional, psychiatric and behavioral functioning. The person in this position will provide early intervention services by establishing a regular presence at District Court hearings and local jails. The Care Coordinator will provide direct support and assistance of a case management nature to consumers. The Care Coordinator will also serve as clinical liaison between mental health providers, court personnel, jail personnel and local law enforcement. The position will provide advocacy to community providers and other stakeholders on behalf of the consumer. The office location is in Chapel Hill, but travel is required to Orange, Person and Chatham counties.

 

HOURS OF WORK:

Normal work hours are 8:00 a.m. – 5:00 p.m. Monday-Friday.

 

QUALIFICATIONS:

Minimal requirements for education and experience are a Master's degree in social work, Licensed or license eligible or a related field with one year of mental health experience, or a Bachelor's degree in social work or a related human services field and three years of social work or counseling experience. Formal study in case management, along with specific work experience related to persons with SPMI, strongly enhances an applicant’s suitability for this position. Incumbent must be able to perform job duties in a culturally competent manner. A valid NC driver’s license with driving record acceptable to OPC insurance carrier is required
